To request a Dutch Hill Maple donation, please submit the below form with your organization/event's information. All donation requests must be completed using this form (no mail, email or phone requests, please). We will review each submitted form and reach out by email within a week, if approved. We love to give back to our community whenever we can, however, due to the large volume of requests received, we are not always able to contribute.

*All approved donation items must be picked up at our farm in Tully.
